Transport of Human Remains

1. Certified copies of Death certificate (3)
2. Burial/Transit Permit
3. Passport and birth certificate of decedent
4. Embalmer's affidavits       (3 - notarized)
5. Letters of non-contagious disease from local health authority (3)
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. Letter confirming that the body is being transferred in a box which is compatible with the international rules
8. A letter from the family asking the body to be transferred back to Turkey
9. Letter showing the airline being used
10. No casket requirement
11. Zinc-lined wood box
12. No fee of inspection
13. 30-45 minute wait at Consul if someone from the funeral home brings documentation otherwise please overnight with a prepaid return overnight envelope
